Daniel Brown, was on the first ship to arrive at the scene of the bombed Air India 747 Jet. Immediately, he became involved in the rescue and subsequent recovery mission. 

​

 Daniel includes video of testimony given at the Commission of Inquiry and Investigation into the event.

The conclusion of the inquiry identified the failure of barriers, compounded by non-compliance with procedures and protocols and highlighted the Human Factors and context of the Decision Making that led to missed opportunities to intervene and affect the eventual disastrous outcome.

"How could this happen?"  A question that was to stay, constant, in the mind of Daniel until the findings of a Royal Commission of Inquiry were published in Ottawa 24 years later.  What was striking about the way in which the event unfolded was how every layer of the safeguard system had Failed, culminating in the destruction of the aircraft and hundreds of lives.   These are systems that are familiar to the workforce across many industries. 

This is not just a story of Air India 182,  not just the story of an Aviation disaster.  It's a story of Failed Barriers, Failed Risk Assessment, Communication, Protocols and Procedures and Inadequate Training and Competency. Human Factors sit menacingly behind each of these operational breakdowns. In sharing this Life Experience and discussing how the event unfolded, Daniel will seek to "Jolt" the Routine and Raise the Awareness of the workforce of the importance in maintaining the integrity of Barriers, Creeping Complacency and Self-Induced Pressure, using the lessons of this disaster to prevent similar tragedies from destroying the lives of others in future.

 This unique presentation holds the audience attention as it explains how the disaster unfolded, highlighting the failings of the systems that were in place at that time and giving context to the Human Factors involved in the Decision Making , the Organisational Culture , Expected Standard and the requirement of separation in Human Performance Competency from Error Traps ,making the workforce think about potential weaknesses in today's Safeguard Systems. 

The report and inquiry into the bombing of Air India Flight 182 is the anatomy of a disaster. This presentation seeks to make linkages to the current activities or philosophies of the attendees, leaving them with an impactful and clear idea of their Risk Management strategies and frontline responsibilities.
